Physician's Affidavit - H. O. Sarber, M.D. (Rockford, Michigan)
Affidavit reads: "I have treated him for Rheumatism of the hips and back, Chronic Prostatitis and Varicocele which is confined mostly to the left testis. On examination I find great tenderness on pressure just below the scrotum, and enlargement of the prostate gland which he claims has troubled him more or less since he came out of the Army. I can not give exact date of first prescription but think about five or six years ago and several times each year since. I believe his trouble was caused by exposure while in the Army. He is unable to perform manual labor more than one half of the time."

Surgeon's Certificate - 25 March 1891 - Original Claim
Certificate reads: We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service, viz: entire loss of left eye, kidney complaint, and rheumatism.
He makes the following statement upon which he bases his claim for original.
The left eye was injured by a limb of a tree striking it, in winter of 1868, and eye was destroyed. He has pain at the root of penis whenever he makes water - this has troubled him for past eight or ten years. Has suffered from rheumatism in shoulders and knees ever since the service. He is totally disabled for manual labor.
Upon examination we find the following objective conditions:
Pulse rate - 62; respiration - 18; temperature - 98.5; height - 5 feet 10 inches; weight - 160 pounds; age - 61 years.
Applicant is a fairly nourished man, hands hard, muscles soft, tongue clean, complexion tanned. The left eye ball is atrophied and shrunken back into the orbit, leaving only a small white stump. He walks bowed over and limps slightly. There is well marked crepitus (Crepitus is a medical term to describe the grating, crackling or popping sounds and sensations experienced under the skin and joints) in both shoulders and knee joints; no deformity nor limitation of motion, except from pain, which is slight. Urine is normal and passes easily into bladder. The prostate gland is considerably enlarged. There is slight varicocele (Varicocele is an abnormal enlargement of the vein that is in the scrotum draining the testicles) of left cord. There are no evidences of his having ever suffered from any venereal disease. In other respects, the soldier is healthy.
He is, in our opinion, entitled to a 17/18 rating for the disability caused by loss of left eye, none for that caused by kidney complaint, and 2/18 for that caused by enlarged prostate; 4/18 for rheumatism.

Certificate reads: We hereby certify that in compliance with the requirements of law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service, viz: Rheumatism - Varicocele and enlarged prostrate and that he receives a pension of twelve dollars per month.
He makes the following statement upon which he bases his claim for additional.
I have had rheumatism since 1862 when I was taken with it in the army. Each year it grows worse until now I am in pain all of the time. In 1864 I fell on the point of my saddle and hurt my testicle. It has pained me ever since. For the past 17 years I have had not use of my tool excepting to make water with. My prostrate has been enlarged ever since I was hurt. I have to get up a dozen times a night to pass water.
Upon examination we find the following objective conditions:
Pulse rate - 72; respiration - 18; temperature - 98.5; height - 5 feet 10 inches; weight - 163 pounds; age - 64 years.
General appearance of claimant is not good. He looks out of health, his muscles are soft and small. His hands do not indicate labor. There is tenderness over both shoulders and hip joints but no deformity. Motion of shoulder joints limited to 3/4. No limitation of motion of hip joints. All other joint ligaments and tendons are normal. There is tenderness over muscles of both shoulders. Also much tenderness over lumbar muscles of both sides. These muscles are also much atrophied. All other muscles are normal.
Tongue is covered with yellow (juice?), tenderness over stomach and bowels. Liver and spleen are normal.
Pulse 72 - 84 -100. Area of cardiac dullness increased in both diameters about one inch. Apex heart is heard in 5th intercostal space just below the nipple. Hearts action is very irregular. There is (?) and hypertrophy of right ventricle. Otherwise heart is normal. There is considerable (?) and vertigo. He has varicocele of left side. Veins are enlarged to four times (thick?) normal size.
Prostate gland enlarged over half. Bladder is somewhat enlarged and its walls are thinned. He complains of burning pain when passing water. There are no indications of any vicious habits. On account of rheumatism - disease of heart - varicocele and enlarged prostate the claimant is unable to perform any manual labor.

Cause of disability: Rheumatism, varicocele and disease of prostate gland.
He receives a pension of twelve dollars per month.
He makes the following statement upon which he bases his claim for increase.
Says he is pensional at $12.00 for loss of eye, that his greatest disability is from disease of testicle, which is painful and nearly disables him for labor, caused by being thrown into his saddle. He also suffers from general rheumatism. Has not been able to work for past 10 years.
We hereby certify that upon examination we find the following objective conditions:
Pulse rate - 90 sitting, 100 standing, 102 after exercise; respiration - 19 sitting, 20 standing, 24 after exercise; temperature - 100; height - 5 feet 10 1/4 inches; actual weight - 152 pounds; age - 70 years.
Pensioner is a (sore?) man, muscles are small, complexion clear, tanned, hands soft, tongue moist & clear, has no teeth, natural or artificial. There is complete collapse of left eye ball, entire destruction of vision, and deformity of orbit. Rt. eye media clear. V. = 20/70. Reads (?) No. VI with +4.88 at normal distance. Rating for left eye seventeen eighteenths (17/18).
There is general muscular and articular rheumatism, muscles are wasted & stiff. There is loud crepitus with impaired motion in all the joints, deposits in finger joints with enlargement, and in both wrists, but none of the joints are completely (?). The cardiac area is enlarged, apex beat is in 6th intercostal space just to left of mammary line. Hearts action is irregular and intermittent, and there is a rough systolic (?) at apex, (next line illegible)...
Rating for rheumatism seventeen eighteenths (17/18), rating for heart disease eight eighteenths (8/18), there is left varicocele, scrotum is pendular, testicle atrophied, and veins of cord enlarged (?) & broken down - tender to manipulation, rt. testicle normal. Rating for varicocele four eighteenths (4/18). Urine is dark (tan?) color, cloudy, contains (?), slightly acid, (?) 1024, no albunim, no sugar, uric acid and urites are (?).
Prostate gland is much enlarged, but urine is passed in good stream. Rating for enlarged prostate four eighteenths (4/18). The lungs, liver, spleen and all other organs are healthy, no other disease, no vicious habits.
